Bridge House - Women's Program - In May 2010 Bridge House will open its Women's Program - expanding the organization's commitment to meet the needs of the grossly underserved female alcoholics and addicts in our community by providing a long-term residential substance abuse recovery program for up to 125 women annually using two effective evidence-based treatment practices: Working Recovery and Gender Specific programming.
Catholic Charities Archdiocese of New Orleans - Case Management Institute - Funding to conduct a market analysis, draft a strategic plan, build an online resource guide, and develop general and advanced training curricula in order to establish the Case Management Institute (CMI). The CMI will draw from local support and expertise to serve as a hub for case management training and resources for the Greater New Orleans community.
Catholic Charities Archdiocese of New Orleans - Hispanic Apostolate - Hispanic Health Promoters Program - This program trains Hispanic Health Promoters and places them at six churches with Hispanic ministries to provide culturally sensitive access to preventive services and medical care for Hispanic parishioners.
Catholic Charities Archdiocese of New Orleans - H.O.P.E. Health Promoters Program: Health Outreach, Promotion and Education - Since Katrina there has been an increase in the Latino population of GNO: its needs are not being met by the system of care. Health Outreach Promotion and Education (HOPE) has been providing assistance to this community for three years. In the next three years HOPE wants to expand to two new areas of need, Medicaid and Food Stamps, and continue its services. HOPE uses health promoters to address their needs in a culturally competent program.
Children's Hospital - Greater New Orleans Immunization Network Booster - The Greater New Orleans Immunization Network has been a successful community outreach program since 1998. GNOIN's mobile unit travels to five parishes and targets the underserved and uninsured population providing free immunizations and raising awareness of their importance.
Common Ground Helath Clinic - Strategic Direction Assessment Project - The Common Ground Health Clinic will conduct a Strategic Direction Assessment. The clinic has transitioned from a disaster relief station to an established primary care facility, and with that evolution comes the need to develop a strategic direction that will ensure the delivery of care to the target population.
East Jefferson General Hospital Foundation - Accelerated Associate of Science in Nursing Program - East Jefferson General Hospital (EJGH) is offering a unique and effective solution to the problems of nursing shortage and lack of professional workforce in the New Orleans area by offering an accelerated program to drive qualified nurses into the workforce as quickly as possible. The result will be an increase in workforce and income generation in the New Orleans area, as well as an augmentation of the nursing workforce to better address the communities' health needs.
Global Maritime Ministries - Port Chaplain Services - Global Maritime Ministries will add a new port chaplain position in order to better meet the physical, social, emotional, and spiritual needs of the rapidly growing maritime community.
Healthy Lifestyle Choices - HLC implements a four-component behavioral health curriculum to reach local children ages 2-12 and provide them with knowledge and skills to make healthy choices before damaging behaviors are initiated.
Kingsley House- Health Care for All Program - The over arching goal of Health Care for All is to improve the physical and mental health of the community. The strategy involves door-to-door outreach and education in a defined community combined with education sessions called Home Health Parties. The Home Health Parties promote nutrition education and mental health topics.
Louisiana Partnership for Children and Families - Positive Parenting Program - Triple P -To support effective parenting in Louisiana, and to improve the health and well-being of all children, the Louisiana Partnership for Children and Families (the Partnership) will facilitate the implementation for an evidence-based parenting program in New Orleans, Triple P. The Partnership will train New Orleans service providers in the Triple P model.
Louisiana Public Health Institute - Assessment of City of New Orleans Community Health Clinics - In partnership with the City of New Orleans, Louisiana Public Health Institute will facilitate a systematic assessment of the function and operating environment of the City of New Orleans' Health Department's community health clinics.
Louisiana Public Health Institute - Greater New Orleans Primary Care Outreach Campaign - The program will apply principles of social marketing to develop a 'new normal' for citizens of New Orleans to access ambulatory primary and behavioral care. The campaign aims to influence health seeking behaviors of routine care while raising awareness of services available close to home. It will include mass media, earned media, a 1-800 number to locate clinics and grassroots community outreach.
L.S.U. Health Sciences - St. Bernard Family Resiliency Project - This program designs, provides and evaluates critically needed evidence based behavioral health services for children and families of St. Bernard Parish who are returning after Hurricane Katrina.
New Orleans Baptist Ministries - Christ Community Health Services, New Orleans Primary Care Project: New Orleans Baptist Ministries will receive funds on behalf of Christ Community Health Services, New Orleans, and develop a strategic plan and offset the startup operational costs and staffing as needed to provide primary health care to serve the residents of the 8th, 9th and Lower 9th Wards.
New Orleans Children's Health Project - The Hispanic Outreach Initiative -The Initiative will allow the New Orleans Children's Health Project to serve New Orleans' growing Hispanic population by providing medical care, mental health care and health education at sites that are convenient to Hispanic patients. Partnerships with other organizations, such as the Hispanic Apostolate and Esperanza Charter School, are fundamental to this initiative. Bilingual staff and Hispanic-specific outreach services will be deployed in late May 2008 to attract over 1,000 Hispanic students and their families to this unique Primary Care Clinic housed in a Mobile Medical Unit to service Esperanza Charter School.
Ochsner Clinic Foundation - Ochsner Health System Chaplain Residency Program - The Ochsner Chaplain Residency Program is a joint project of Ochsner and the McFarland Institute (TMCFI) to provide professional education for ministry through a full-time residency in pastoral care. During the start-up program, Ochsner and TMCFI will train 13 chaplains and pursue accreditation for Ochsner as an Accredited CPE Center through the Association for Clinical Pastoral Education
Responsibility House - Integrated Treatment Training and Education Program - Responsibility House will provide academic and clinical training programs on integrated treatment of co-occurring substance abuse and mental disorders. THe program will be developed and operated in partnership with and replicated at local universities. With better prepared clinicians in the community, treatment options for the dually-diagnosed will improve.
St. Tammany Parish Hospital Foundation - Nurse Family Partnership Mental Health Services - Eleven percent of infants living in poverty have a mother suffering from depression. The Nurse Family Partnership (NFP) will identify depressed first-time mothers living in poverty, provide evaluations and referrals for mental health services, and provide follow-up services. The program will reduce the number of suicides among first-time mothers and improve developmental outcomes for children.
St. Thomas Community Health Center - Pediatric Healthcare Services in New Orleans, LA. - St. Thomas Community Health Center will partner with the Mahalia Jackson School to provide primary health care services to children enrolled at the daycare center. Infrastructure is in place thanks to an existing community health center at the facility. This funding will provide for a nurse practitioner and pediatrician to staff the health center and provide direct patient care.
Second Harvest Food Bank of Greater New Orleans - 9-A-Day the Head Start Way - With the goal of ending childhood hunger and improving nutrition in our community, Second Harvest developed the Nine-A-Day Program to provide low-income, pre-school children with fresh produce and the skills and knowledge to adopt lifelong healthy eating habits. Nine-A-Day will serve more than 1,000 children, their families and staff members at 12 Head Start sites in Orleans and Jefferson parishes
Seton Resource Center for Child Development - Human services Program - This program will provide a comprehensive behavioral health program to two elementary schools in New Orleans.
Services include:
- preventative and early intervention programs
- individual and group counseling
- filial therapy and family counseling,
- referral services for children and their families.
Sisters of Mercy Ministries - Project Fleur-de-lis - Project Fleur-de-lis (PFDL) was created to address the unmet mental health needs of school-aged children in the Greater New Orleans area. PFDL is a collaborative program linking social service agencies, schools and nationally recognized researchers, program developers and clinicians in a coordinated effort to provide evidence-based mental health services in our school communities.
Trinity Church - Strategic Direction Assessment for the Trinity Counseling and Training Center: Trinity Church will receive funds to conduct a Strategic Direction Assessment Report for the Trinity Counseling and Training Center to analyze and make recommendations on the Center's ability to provide quality mental health services to the predominantly low-income target population it serves within its fiscal limitations.
Tulane University Health Science Center - A Sickle Cell Day Hospital - This project proposes to establish a day hospital for adults with sickle cell disease to provide immediate and appropriate care for patients having uncomplicated painful episodes; thereby reducing ER visits, hospitalization, saving money and more importantly reducing morbidity.
