The implementation manual cur- rently under development is a big step toward that objective, along with ongoing data gathering. Jefferson and Beyond From BCM’s perspective, JPHSA’s work toward centralized care coordination wraps around a bold vision: that patients in Jefferson Parish have the best healthcare options possible, and that one day the same may be true for all of Louisiana.
